And not because narrow specialization is insufficient, but quite the opposite. POCUS is felt only by clinicians who can see patients in their disease dynamics every week. Therefore, POCUS is taught by diagnostic and therapeutic practitioners: from metropolitan doctors to rural family doctors. The curse of ultrasound experts is their knowledge, and the strength of clinicians is the understanding of pretest probability, distinguishing patterns, and the ability to track ultrasound dynamics.
Clinicians know that there are many other approaches besides standard positions. It is not always necessary to measure the aorta, as it can be compared with the vertebral body. Narrow specialists measure the volume of effusion, while clinicians align it with the clinical practice: because cardiac tamponade is not only about volume but also about Beck’s triad....
That is why IEU trainings are attended by more and more practitioners interested not in the certificate but in the daily use of the ultrasound method to answer a specific clinical question. We are glad that the quarter of the participants are rural family doctors who understand that POCUS means bringing care closer to remote locations.
